Job summary

Theoria Medical is seeking a part-time Medical Director and Attending Physician to serve local skilled nursing facilities. You will deliver patient-centered care as the attending physician, collaborate with NPs/PAs, and participate in care management programs.

Qualifications

• MD/DO with an active U.S. medical license.
• Board-certified or eligible in a suitable specialty.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, and Geriatrics.

Responsibilities

• Serve as the attending physician for patients in each assigned SNF.
• Collaborate with NPs/PAs to manage daily patient care and supervision.
• Provide high-quality primary care, diagnosing and treating acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring, and other quality initiatives.
