Theoria Medical seeks a part-time Medical Director and Attending Physician to cover local skilled nursing facilities. You will build trust with residents, provide continuous care, and enjoy schedule autonomy with a collaborative clinical team behind you.

Qualifikationen

• MD/DO with active US medical license.
• Board-certified in internal medicine, family medicine, emergency medicine, geriatric medicine or related specialties.
• New graduates who are board-eligible are encouraged to apply.

Aufgaben

• Serve as the attending physician for patients in assigned SNFs.
• Collaborate with NPs/PAs to manage daily patient care and provide clinical supervision.
• Deliver high-quality, patient-centered primary care, including diagnosing and treating acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring and other quality initiatives.
