Job summary

Theoria Medical is seeking a part-time Medical Director and Attending Physician to oversee patient care at local skilled nursing facilities. You will lead a collaborative clinical team, support NPs/PAs, and exercise autonomy over your schedule to focus on the patient in front of you.

Qualifications

• MD/DO with active U.S. medical license and board certification.
• New graduates who are board-eligible are encouraged to apply.
• Specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, PM&R, and Geriatrics.

Responsibilities

• Serve as the attending physician for patients in assigned SNFs.
• Collaborate with NPs/PAs to manage daily patient care and supervise clinicians.
• Provide high-quality primary care, diagnosing and treating acute and chronic conditions.
• Participate in Chronic Care Management, Transitional Care, Remote Patient Monitoring and related quality initiatives.
