You will collaborate with the nursing staff, participate in care planning, ensure timely MDS completion, and support infection control processes while adhering to federal, state, and local regulations. This role emphasizes leadership, teaching, and accurate documentation for quality patient care.

Qualifications

• Current state licensure (RN or LVN)
• Completed nursing program with degree from accredited college/university
• Preferably 1 year in resident assessment position
• Progressive nursing experience with leadership and teaching abilities
• Demonstrated planning, organizing, and directing skills
• Strong communication, oral and written
• Knowledge of elder/chronically ill care needs
• Ability to teach adult learners
• Basic computer skills
• Infection control committee principles

Responsibilities

• Conduct and collect information to develop/complete MDS per rules and regulations
• Participate in surveys by government agencies
• Work with IDT to develop resident assessment and care plan
• Evaluate resident conditions and medical data for assessment changes
• Schedule participation by resident's representative and family
• Plan and deliver in-service training on assessment skills
• Ensure completeness/accuracy of assessments and awareness of penalties for false certification
• Coordinate development of written care plans with problems, goals, and responsible services
• Ensure measurable objectives and timetables for medical/nursing/psychosocial needs
• Complete initial assessment within RAI timeframe
• Complete comprehensive care plan within 21 days of admission
• Encourage resident/family participation in plan development/review
• Ensure all MDS assessments and care plan reviews are timely
• Complete assessments within 14 days of significant change
• Transmit MDS assessments to appropriate agencies
• Attend departmental meetings and in-service training
• Perform other related assignments
