Job summary

Bethea Retirement Community, now managed by Oakdale Seniors Alliance in Darlington, SC, seeks an organized MDS Coordinator (LPN) to lead the MDS process and ensure accurate reimbursement. Remote work not offered; day shift and full-time hours apply.

The role requires an active SC LPN license and prior MDS coordination experience in SNF/long-term care, with strong CMS knowledge and EHR proficiency.

Qualifications

• Active, unencumbered LPN license in SC (or compact state) required.
• Prior MDS Coordinator experience in SNF/Long-Term Care required.
• Deep knowledge of CMS regulations, RAI manuals, PDPM reimbursement, and state survey processes.
• Experience with EHR systems (PointClickCare, MatrixCare) and MDS transmission.

Responsibilities

• Manage the MDS process and reimbursement accuracy.
• Coordinate OBRA and PPS assessments within federal/state timeframes.
• Lead interdisciplinary meetings and care planning with IDT (nursing, therapy, social services).
• Monitor CMS compliance and ensure audit-ready documentation.
• Transmit MDS data to state databases and monitor trend indicators.
