Job summary

Southpoint Rehabilitation and Healthcare Center in North Carolina is seeking a Part-Time MDS Coordinator to ensure compliant, resident-centered care planning and accurate documentation across the facility.

You will stay current with federal and state regulations, coordinate with nursing leadership, oversee discharge processes and audits, and maintain confidentiality and safety in all MDS activities while supporting staff development.

Qualifications

• Current state license as a Nurse is required.
• CPR certification is required.
• Previous experience as an MDS nurse is required.

Responsibilities

• Keep abreast of current federal and state regulations and professional standards.
• Assist with development of comprehensive care plans for Residents in coordination with the MDS.
• Coordinate, manage and monitor the written plan of care for each Resident.
• Assist nursing management with coordination, management, and review of nurse's notes.
• Monitor Resident status changes to ensure appropriate nursing or clinical involvement.
• Ensure MDS and support documentation meet regulatory and auditor requirements.
• Perform regular audits of documentation for accuracy.
• Assist nursing management with the discharge process.
• Complete administrative forms and reports.
• Maintain infection control and safety precautions; protect confidentiality.
