Southpoint Rehabilitation and Healthcare Center in Southpoint, NC, is hiring a Part-time MDS Coordinator to join our dedicated team. You will coordinate MDS assessments, develop and monitor care plans, and ensure the written plan of care reflects residents’ needs and goals while maintaining regulatory compliance.

Ideal candidates hold a current nursing license, CPR certification, and prior MDS experience, with strong documentation and communication skills.

Qualifications

• Current state nursing license (RN or LPN).
• CPR certification required.
• Prior experience as an MDS nurse preferred.

Responsibilities

• Maintain MDS documentation and ensure regulatory compliance.
• Coordinate, manage and monitor the written plan of care for each resident.
• Audit resident records for accuracy.
• Assist nursing management with discharge planning.
• Ensure confidentiality and safety standards are followed.
• Attend departmental meetings and in-services.
