Southpoint Rehabilitation and Healthcare Center in Southpoint, NC is seeking a part-time MDS Coordinator to join our care team. You will ensure accurate MDS planning and regulatory compliance while coordinating with nursing and clinical staff to support resident needs.

Responsibilities include maintaining care plans, auditing documentation, and assisting with discharges in a supportive, patient‑centered environment.

Qualifications

• Knowledge and training in all aspects of MDS process.
• Ability to stay updated on federal and state regulations affecting MDS.
• Experience coordinating care plans and MDS documentation.

Responsibilities

• Keep abreast of current federal and state regulations and standards.
• Develop comprehensive care plans with MDS input to address resident needs.
• Coordinate and monitor the written plan of care for each resident.
• Review nurse notes to ensure care plan compliance.
• Monitor resident status changes and involve clinical team promptly.
• Maintain accurate MDS documentation and regulatory compliance.
• Assist with discharge processes and administrative tasks.
• Maintain infection control and patient confidentiality.
