Qualifications

• Knowledge of MDS process and state/federal regulatory requirements.
• Experience coordinating care plans and multidisciplinary input.
• Strong communication and documentation skills.

Responsibilities

• Keep abreast of current federal, state regulations and professional standards.
• Assist with development of comprehensive care plans with the MDS.
• Coordinate and monitor written plan of care for residents.
• Assist nursing management with care plan compliance review.
• Monitor resident status changes for timely involvement.
• Ensure MDS documentation accurately reflects residents.
• Perform audits of documentation for accuracy.
• Assist with discharge processes.
• Complete necessary forms and reports.
• Uphold infection control and safety practices.
