Qualifications

• 1-3 years of MDS experience.

• Current and unencumbered LVN/LPN license in California.
• Knowledge of clinical standards of practice, regulations, and Medicare participation.
• Regulatory guidelines for long-term care and Medicare participation.

Responsibilities

• Assist in completing the Minimum Data Set (MDS) assessment for residents.
• Coordinate the MDS process ensuring timely submission per state/federal regs.
• Collaborate with interdisciplinary teams to support care planning with data.
• Maintain accuracy and integrity of resident information in the system.
• Monitor resident status and update MDS assessments as needed.
• Ensure compliance with Medicare participation and regulatory requirements.
