Job summary

The Earlwood Care Center is seeking a Part-Time Minimum Data Set Assistant (MDS Asst-LVN/LPN) to join our care team in Torrance, CA. The role focuses on coordinating and completing MDS assessments to support resident care and regulatory compliance.

Responsibilities include data collection, interdisciplinary collaboration, and timely submission of MDS documents, with emphasis on accuracy, privacy, and adherence to state and federal requirements.

Qualifications

• 1–3 years of MDS experience.
• Nursing degree from an accredited college.
• Current and unencumbered LVN/LPN license in the state.
• Knowledge of long-term care regulations and Medicare participation.

Responsibilities

• Assist in the completion of the Minimum Data Set (MDS) assessment for all residents.
• Coordinate the MDS process, ensuring accurate and timely submission in accordance with state and federal regulations.
• Collaborate with interdisciplinary teams to gather necessary data and support the resident’s care planning process.
• Maintain accuracy and integrity of resident information by ensuring all data is entered correctly.
• Monitor resident status and changes that may require updates to MDS assessments.
• Assist with compliance with the regulatory requirements governing MDS, including Medicare participation.
