Job summary

BayMark Health Services, Inc. is seeking a Part-Time Patient Navigator to support MAT linkage and outreach across EDs, jails, and other settings in the Sacramento area.

The role involves coordinating referrals, educating external partners, and ensuring patients access MAT and follow-up care. Responsibilities include conducting rounds, maintaining referral data, and assisting with insurance status, financial counseling, and appointment scheduling.

Qualifications

• Must have nonjudgmental, energetic and positive approach to assisting patients with SUD (substance use disorder).
• Understanding of SUD as a disease and the role of MAT in OUD treatment.
• Excellent oral and written communication skills; proficient in MS Office (Word, Excel, Project).
• Proven ability to meet deadlines with strong analytical and organizational skills across multiple tasks.
• Ability to work in a fast-paced environment with attention to audit trails and detail.
• Ability to build positive rapport with clients, vendors, and staff; maintain partnerships.
• Compliance with professional standards and practices.
• Demonstrates initiative to handle unforeseen situations and adapt to schedule changes.
• References satisfactory from employers or peers; background and drug screening satisfactory.

Responsibilities

• Assist TCD to develop outreach and education lists.
• Provide education and outreach to external entities developed with TCD.
• Round in ED, jail and other settings to identify patients eligible for MAT.
• Maintain data for referrals and outreach/education activities.
• On-call for MAT linkage during regular hours if not present in ED or jail.
• Describe program and OUD MAT principles to external entities.
• Obtain consent and complete MAT intake forms.
• Assess readiness using the Stages of Change model.
• Assess for co-occurring disorders and medical conditions.
• Confirm and assist with insurance status; connect uninsured patients to financial counseling.
• Identify resources for OUD MAT and assist with follow-up appointments.
• Address barriers to attendance (transportation, contact info, shelter, etc.).
• Communicate with ED/jail providers about MAT follow-up plans and schedules.
• Contact patients the day before follow-up appointments.
