Job summary

The MAT Navigator at Central Neighborhood Health Foundation’s Grand Avenue Clinic in Los Angeles provides outreach and education about MAT programs. You will engage with the community, serve as a primary contact for patients in the Substance Use Disorder Program, and coordinate care with clinicians and community resources.

Qualifications

• Community Health Worker with 1 to 2 years’ experience in clinical or community resource settings.
• Care coordination and/or case management experience.
• Evidence of essential communication, education, and counseling skills.
• Proficiency in communication technologies (email, cell phone, etc.).
• Highly organized with ability to keep accurate notes and records.
• Experience with health IT systems and reports is desirable.
• Local knowledge about community health care and social welfare resources is desirable.
• Knowledge of SUD treatment and targeted populations is desirable.

Responsibilities

• Identify prospective patients and explain MAT program to the community.
• Provide MAT education and outreach; act as primary contact for SUD program participants.
• Coordinate with clinic staff, providers, and community resources to support MAT services.
• Travel to other CNHF clinics to support MAT services.
• Advocate for MAT expansion and serve as information resource.
