Job summary

Central Neighborhood Health Foundation seeks a MAT Navigator at the Grand Avenue Clinic in Los Angeles to engage the community, explain the MAT program, provide education and outreach, and act as the primary contact for patients enrolled in the Substance Use Disorder Program.

This role supports addiction medicine providers, coordinates care, travels to CNHF clinics, builds partnerships with community resources, and helps patients access MAT services and transportation.

Qualifications

• Experience as a Community Health Worker in clinical or community settings.
• Care coordination or case management experience.
• Strong communication, education, and counseling skills.
• Proficiency with email and mobile communication.

Responsibilities

• Engage community to identify prospective patients and explain MAT program.
• Provide MAT education and outreach; act as focal contact for MAT services.
• Coordinate with clinicians and community resources to support care transitions.
• Travel to CNHF clinics to support MAT services and outreach.
• Monitor patient progress and assist in adherence to MAT grant requirements.
