Qualifications

• BLS and Heartsaver/CPR certifications preferred.

Responsibilities

• Perform screenings, intake assessments and consultations for evaluation of individual and family dynamics and treatment needs.
• Provide psychosocial interventions and education to patients, families and significant others.
• Utilize individual, family, group work and crisis intervention methods for treatment and discharge planning.
• Complete comprehensive psychosocial histories on a timely basis.
• Provide direct treatment services including individual, family, and group counseling and crisis intervention.
• Serve as liaison to healthcare team regarding social work issues and community resources.
• Assist in developing discharge and post-hospital care plans and community linkages.
