Job summary

Bergen New Bridge Medical Center is seeking a Licensed Social Worker for an on-site, full-time role in Paramus, NJ. You will perform screenings, assessments, case management, and direct psychosocial treatment for patients and families, coordinating discharge planning and community referrals.

Qualifications

• Master’s degree in social work from an accredited school.
• An LSW license is required within one year of hire.
• BLS/Heart Saver or CPR certifications preferred.

Responsibilities

• Perform screenings, intake assessments and referrals for evaluation of individual and family dynamics and treatment needs.
• Provide psychosocial interventions to patients and families, including education.
• Utilize individual, family, group counseling and crisis intervention methods.
• Complete psychosocial histories on a timely basis.
• Provide case management to ensure treatment plans are followed and linkage to resources.
• Act as liaison to health care team regarding family dynamics and discharge planning.
• Advise on community resources and assist in post-hospital care planning.
• Maintain documentation and participate in departmental meetings and committees.
