Summit Home Care and Hospice is seeking an experienced LPN to join our Home Health team in Washington, IN. This part-time role offers potential transition to full-time, with on-the-road patient visits and occasional remote work at the local office. Mileage is reimbursed, and pay ranges $28.00–$31.00 per hour.

The LPN must be a graduate of an accredited program with Indiana licensure. 3 years of nursing experience is preferred, with emphasis on home health or medical/surgical settings.

This LPN position will start as a part-time role with the potential to transition to a full-time position.**Licensed Practical Nurse (LPN):*** **Part-Time Hours:**Mondays, Wednesdays Fridays (3 Days/Week); Standard Business Hours* **Work Setting:**On the Road for Patient Visits & Training at the Local Office or Remotely* **Travel:**Required + Mileage Reimbursed* **Pay Rate:** $28.00 - $31.00 per Hour* **Location:**Washington, IN & Surrounding Areas**Responsibilities:*** Provide direct patient care under the supervision of a registered nurse.* Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.* Implement the plan of care initiated by the registered nurse.* Provide accurate and timely documentation consistent with the plan of care.* Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.* Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.* Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.* Participate in personal and professional growth and development.* Perform other duties assigned by the registered nurse.* Participate in weekend and holiday on-call rotation requirements.## Qualifications**Qualifications:*** Must be a graduate from an accredited practical nurse or vocational nursing program.* Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Indiana.* 3 years of nursing experience, preferred.* Prior experience in community/home health and/or medical/surgical, preferred.* Comply with accepted professional standards and practices.* Demonstrates good verbal and written communication, and organizational skills.* Must be able to lift, position or transfer patients and lift supplies and equipment.**Why Summit?**We offer a well-rounded benefits package to support you both at work and at home.
