Summit Home Care and Hospice is seeking an experienced LPN to join our Home Health team. The role starts part-time with potential to transition to full-time, performing patient visits and on- site training as needed.

Responsibilities include direct patient care under RN supervision, following and implementing the care plan, timely documentation, and patient/family education. Travel is required with mileage reimbursement; office or remote training may occur.

Qualifications

• Graduate from an accredited practical nurse or LVN program.
• Hold a valid Indiana LPN/LVN license.
• 3 years nursing experience preferred.
• Community/home health or medical/surgical preferred.
• Strong verbal and written communication and organizational skills.
• Able to lift, position or transfer patients and equipment.

Responsibilities

• Provide direct patient care under supervision of a registered nurse.
• Follow the plan of care, deliver treatments and collaborate with the team for positive outcomes.
• Implement the plan of care initiated by the RN.
• Document accurately and timely per the plan of care.
• Educate patients and families on self-care and relevant information.

• Use and prepare equipment; adhere to aseptic techniques.
• Participate in weekend and holiday on-call rotation.

Summit Home Care and Hospice is searching for an experienced and motivated LPN to join our Home Health team. This LPN position will start as a part-time role with the potential to transition to a full-time position.

Licensed Practical Nurse (LPN)

• Part-Time Hours: Mondays, Wednesdays Fridays (3 Days/Week); Standard Business Hours
• Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely
• Travel: Required + Mileage Reimbursed
• Pay Rate: $28.00 - $31.00 per Hour
• Location: Washington, IN & Surrounding Areas

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.
• Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.
• Participate in personal and professional growth and development.
• Perform other duties assigned by the registered nurse.
• Participate in weekend and holiday on-call rotation requirements.

Qualifications

Qualifications:

• Must be a graduate from an accredited practical nurse or vocational nursing program.
• Must hold a valid/active license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Indiana.
• 3 years of nursing experience, preferred.
• Prior experience in community/home health and/or medical/surgical, preferred.
• Comply with accepted professional standards and practices.
• Demonstrates good verbal and written communication, and organizational skills.
• Must be able to lift, position or transfer patients and lift supplies and equipment.
