Job summary

Summit Home Care and Hospice is seeking an experienced LPN to join our Home Health team in Washington, IN. This part-time role may transition to full-time, with travel to patient visits and training at the local office or remotely. You will provide direct patient care under RN supervision and participate in care coordination and documentation.

The position emphasizes adherence to the plan of care, patient and family education, and weekend/holiday on-call rotation as needed.

Qualifications

• Graduate from an accredited practical nurse or vocational nursing program.
• Active Indiana LPN/LVN license.
• 3 years of nursing experience preferred.
• Home health or community-based experience preferred.
• Adheres to professional standards and practices.

Responsibilities

• Provide direct patient care under the supervision of a registered nurse.
• Follow the plan of care, provide treatments, and collaborate with the team to meet patient outcomes.
• Implement the plan of care initiated by the RN.
• Provide accurate and timely documentation consistent with the plan of care.
• Educate patients and families on self-care per diagnosis and plan of care.
• Coordinate home health services and report needs to RN/Clinic Manager.
• Prepare equipment and supplies, adhere to aseptic techniques.
• Engage in personal and professional growth and development.
• Perform other duties assigned by the RN.
• Participate in weekend and holiday on-call rotation requirements.
