Job summary

Oakview Health and Rehabilitation in Summerville, GA is seeking a licensed practical nurse (LPN) or licensed nurse with a nursing degree to support patient assessments and care planning. You will coordinate with interdisciplinary teams, assist with interviews and evaluations, and help ensure timely, compliant care plans in a supportive, team-oriented environment.

You should have an active Georgia LPN license, strong communication skills, and proficiency with Microsoft Office and Excel.

Qualifications

• LPN or nursing degree from an accredited program; active Georgia license.
• Proficient in Microsoft Office (Outlook, Word) and Excel.
• Strong organizational, written and verbal communication skills.

Responsibilities

• Facilitates communication between nursing center departments and patient/family to support care plans.
• Interacts with consultants and interdisciplinary team to ensure comprehensive care.
• Contributes to assessment/care plan development and scheduling.
• Ensures care plans align with regulations and patient wishes.
• Assists in education of staff on care plans and MDS-related activities.
