Job summary

Qualifications

• High School Diploma or GED; nursing certificate as applicable, and LPN license required upon hire.
• BLS certification required (or equivalent) for access to patient care.
• Healthcare experience preferred; oncology/ambulatory settings preferred.

Responsibilities

• Provide direct patient care under RN supervision and within LPN scope.
• Perform routine nursing procedures and monitor patient condition.
• Coordinate tests, treatments and follow-up care with the care team.
• Document all patient care activities accurately in medical records.
• Educate patients and families to support self-care and treatment plans.
• Participate in staff meetings, in-services, and ongoing education.
