Job summary

Qualifications

• LPN license required upon hire.

Responsibilities

• DIRECT PATIENT CARE: Assess and assist with patient needs under RN oversight and nursing process.
• Perform routine nursing procedures under RN supervision.
• Coordinate care and services with RN and team members.
• PATIENT / CAREGIVER EDUCATION: Educate patients and caregivers for self-care.
