Job summary

Genesis HealthCare, Inc. is seeking a compassionate LPN for our Assisted Living community in Hampton, NH. This part-time role (11P-7A) focuses on building relationships while delivering skilled nursing under RN supervision to support residents’ vitality and independence.

You will document patient data, participate in care planning, administer medications, and ensure clear communication with the RN and physicians. Join a team dedicated to high-quality care and resident well‑being.

Qualifications

• Graduate of an approved school for practical or vocational nursing.
• Current LPN license by the State Board of Nursing in the state where the center is located.
• Read, write, speak and understand English to ensure safety and care.
• TST verification as required by state law and Company policy.
• BLS/CPR certification current as required by state.

Responsibilities

• Collect, report and document patient data; observe and report changes to the RN.
• Contribute to individualized care plans with goals and interventions.
• Administer medications and perform treatments as ordered by physicians.
• Communicate pertinent patient data to the RN and/or physician.
• Monitor and supervise care provided by unlicensed staff per policies.
• Use PCC documentation and maintain accurate drug counts.
