Job summary

All American Assisted Living at Londonderry is seeking a Part-Time LPN to join our Wellness team. The role focuses on coordinating resident care, administering medications, monitoring changes, and supporting the care team under nursing leadership.

The LPN will provide hands‑on nursing, participate in assessments, and help implement individual service plans while maintaining resident dignity and safety in a resident‑centered community in New Hampshire.

Qualifications

• Active LPN license in New Hampshire or multistate license with NH authorization.
• License must be active and in good standing.
• Ability to communicate in English clearly and professionally.
• Strong documentation and time management skills.

Responsibilities

• Administer medications and treatments per orders and policies.
• Monitor residents for changes in condition and report promptly.
• Provide direct nursing care and support to residents.
• Assist with assessments and service-plan reviews.
• Maintain accurate documentation of meds, observations, and services.
• Coordinate with families, providers, and pharmacy staff.
• Support admissions, transitions, and changes in condition.
• Participate in resident-care meetings and educate residents and families.
