Lowe’s Guardian Angel in Georgia is seeking a Part-time Licensed Practical Nurse (LPN) to coordinate medical care and provide clinical oversight for individuals with IDD in residential and day-support settings. The role emphasizes accurate documentation, collaboration with care teams, and adherence to DBHDD standards.

The LPN will monitor and coordinate medical services, support triage procedures, and assist with ISP/SIS meetings.

Competenze

• Active LPN license in Georgia.
• CPR/BLS certification required.
• Experience with individuals with IDD/behavioral health needs preferred.
• Knowledge of DBHDD standards and MAR/HRST documentation is beneficial.

Mansioni

• Coordinate medical care and clinical oversight for IDD/dual-diagnosis individuals in residential/day-support settings.
• Complete and oversee nursing documentation and progress notes.
• Coordinate medical appointments and transportation for medical services.
• Audit medication administration records and ensure proper medication handling.
• Provide staff training on medical management and QA processes.
• Attend ISP/SIS meetings and support intake/transition processes.
