StoneBridge Jacksonville is seeking a compassionate Licensed Practical Nurse (LPN) to join our resident-centered care team. You will administer medications, coordinate nursing care on the unit, and support residents with dignity in a long-term care setting.

Applicants should hold a current LPN license, CPR certification, and have basic computer skills with EMR experience. Previous LTC experience preferred; training and teamwork are highly valued at StoneBridge Jacksonville.

Qualifications

• Must have a current state LPN license.
• CPR certification required; nursing specialty certification preferred.
• Minimum 2 years of experience in long-term care.
• Basic computer skills and EMR experience.

Responsabilités

• Coordinate nursing care on the unit, ensuring residents have a nurse aide.
• Administer and document medications and treatments per schedule.
• Perform nursing assessments and notify physician of changes.
• Prepare work assignments considering safety and staff capabilities.
• Assist residents and respond to requests and pain management.
