Job summary
Executive Home Care of Augusta is seeking a compassionate Licensed Practical Nurse (LPN) for a part-time role in Augusta, GA. The position emphasizes relationship-based, one-client-at-a-time care and daytime availability.
You will conduct nursing visits, monitor health, reconcile medications, educate on chronic disease management, and support care plans while mentoring the caregiving team in a veteran-owned agency dedicated to seniors living safely at home.
Qualifications
- Current Georgia LPN license in good standing.
- Minimum one year of nursing experience (home care, geriatrics, hospice, assisted living, rehabilitation, or skilled nursing preferred).
- Strong assessment and communication skills.
- Compassionate, patient-centered approach.
- Reliable transportation and valid driver's license.
- Ability to pass background screening and drug screening.
- Current CPR certification.
Responsibilities
- Conduct nursing and wellness visits.
- Monitor client health status and document observations.
- Perform medication reconciliation and education.
- Assist with chronic disease education and wellness coaching.
- Communicate changes in client condition to physicians and family members.
- Support care plan development and updates.
- Train and mentor caregivers on client-specific needs.
- Assist with hospital-to-home transitions.
- Maintain accurate clinical documentation.
- Collaborate with Care Managers to ensure outcomes.
