Qualifications

• Current Georgia LPN license in good standing.
• Minimum one year of nursing experience (home care, geriatrics, hospice, assisted living, rehabilitation, or skilled nursing preferred).
• Strong assessment and communication skills.
• Compassionate, patient-centered approach.
• Reliable transportation and valid driver's license.
• Ability to pass background screening and drug screening.
• Current CPR certification.

Responsibilities

• Conducting nursing and wellness visits.
• Monitoring client health status and documenting observations.
• Performing medication reconciliation and medication education.
• Assisting with chronic disease education and wellness coaching.
• Communicating changes in client condition to physicians and family members.
• Supporting care plan development and updates.
• Training and mentoring caregivers on client-specific care needs.

• Maintaining accurate clinical documentation.

Executive Home Care of Augusta is a veteran-owned, concierge-level home care agency seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our team. We help seniors across the CSRA maintain their independence and remain safely at home — and we do it through personalized, unhurried, relationship-based care, not a rotating caseload.

What You'll Do
