You will deliver 1:1 nursing care, monitor conditions, administer medications, and support care plans within a flexible schedule that includes day, evening, and overnight shifts.

Qualifications

• Active LPN license.
• Current BLS certification.
• 1+ year nursing experience preferred.

Responsibilities

• Provide direct patient care in the home.

• Support individualized care plans.
• Document care and communicate with the healthcare team.
