Job summary

Jewish Family & Career Services of Atlanta (JF&CS) is seeking a qualified Licensed Practical Nurse (LPN) to join our team. The role focuses on nursing administration, health documentation, and care coordination for adults with intellectual and developmental disabilities.

The part-time position involves 20 hours per week with a flexible schedule, including some evenings and weekends for client home visits. Local travel to group homes is required, with mileage reimbursement where applicable.

Qualifications

• Active Georgia LPN license in good standing (required).
• Valid Georgia driver’s license in good standing (required).
• Able to complete 16 hours of annual training required by DBHDD.
• Experience with electronic health record documentation (required).
• Strong interpersonal and communication skills.
• Excellent organizational skills.
• Ability to work independently and as part of a team.

Responsibilities

• Maintain organized, up-to-date health records and care plans.
• Coordinate scheduling for home visits and appointments.
• Support documentation for Medicaid waiver, CARF, and DBHDD requirements.
• Track health care protocols and flag updates for RN review.
• Assist with client care monitoring and routine home visits every 60 days.
• Provide backup on-call nursing coverage as scheduled.
