Job summary

Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Independent Social Worker (LISW) to join an integrated clinical team onsite at one or more long-term care facilities in your local community. You will provide individual, family, and group therapy, conduct assessments and care planning, and support residents with depression, anxiety, and other geriatric concerns.

The role offers autonomy with a Monday–Friday schedule and strong administrative support.

Qualifications

• Active LISW license.
• Experience providing therapy in geriatric or LTC settings.
• Ability to collaborate with interdisciplinary teams.
• Strong documentation and care planning skills.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Coordinate with nursing, social services, rehabilitation, and medical providers.
