Deer Oaks Behavioral Health — Long-Term Care Clinical Services is hiring a Licensed Independent Social Worker (LISW) for full-time work in one or more local long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support on a Monday–Friday schedule.

Qualifications

• Active LISW license and eligibility to practice in Ohio.
• Excellent written and verbal communication, and interpersonal skills.
• Ability to work independently within an interdisciplinary team.
• Strong documentation and organizational abilities.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, and treatment planning.
• Coordinate with nursing, social services, rehab, and medical staff.
• Maintain compliant, timely documentation using GNT.
