Deer Oaks Behavioral Health — Long-Term Care Clinical Services is seeking a Licensed Clinical Social Worker (LCSW) to join an integrated clinical team onsite at one or more long-term care facilities. You will provide therapy, assessments, care planning, and behavioral health support for residents, with a Monday–Friday schedule and strong clinical and administrative backing.

Qualifikationen

• Active, unrestricted LCSW license.
• Reliable driver's license and transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Aufgaben

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, and other LTC needs.
• Deliver behavioral management and goal-oriented interventions.
• Coordinate treatment with nursing, social services, rehabilitation, and medical providers.
• Participate in care plan discussions and facility meetings as needed.
• Maintain complete documentation using the Geriatric Notes Tool (GNT).
• Build strong relationships with facility staff and leadership.
