Job summary

Deer Oaks is seeking a licensed clinical social worker to join our geriatric behavioral health team in long-term care facilities. You will provide therapy, conduct assessments, participate in care planning, and support residents’ mental health on-site with a collaborative interdisciplinary team.

This role offers Monday–Friday scheduling, autonomy in workflow, and strong administrative and clinical support to focus on delivering compassionate care to older adults.

Qualifications

• Active, unrestricted LCSW license.
• Valid driver’s license and reliable transportation.
• Strong written, verbal, and interpersonal communication skills.
• Ability to work independently while collaborating with an interdisciplinary team.
• Commitment to timely documentation and high-quality care.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments, psychosocial evaluations, screenings, and treatment planning.
• Support residents with depression, anxiety, adjustment to illness, grief, trauma, and behavioral issues.
• Maintain complete and timely documentation using the GNT and follow HIPAA guidelines.
• Collaborate with nursing, social services, rehab, and medical staff to coordinate care.
