Job summary

Deer Oaks is seeking a Licensed Clinical Social Worker (LCSW) to join our integrated clinical team in one or more long-term care facilities. You will deliver therapy, conduct assessments, and develop care plans while collaborating with an interdisciplinary team to improve resident outcomes.

The role emphasizes autonomy with consistent administrative and clinical support, a Monday–Friday schedule, and a focus on geriatric behavioral health for residents with complex medical needs.

Qualifications

• Active, unrestricted LCSW license.
• Valid driver’s license.
• Ability to work independently while collaborating with an interdisciplinary team.
• Experience with geriatric populations preferred.

Responsibilities

• Provide individual, family, and group therapy to LTC residents.
• Conduct clinical assessments and psychosocial evaluations; develop treatment plans.
• Collaborate with nursing, social services, rehab, and medical teams.
• Maintain compliant documentation in the GNT and follow HIPAA guidelines.
• Build strong relationships with facility staff and leadership.
