Job summary

Vibra Healthcare, LLC. is seeking a Part Time Social Worker to join our care team in a hospital setting. The role focuses on patient assessment, counseling, and discharge planning, while coordinating with families, hospital staff, and community resources to support recovery and safe transitions.

You will work within an interdisciplinary team including physicians, nurses, and case managers, applying psychosocial approaches to improve patients' social functioning and independence.

Qualifications

• Bachelor’s Degree in Social Work required.
• Current, valid Social Worker license in the employment state required.
• Two years of healthcare social work experience preferred.

Responsibilities

• Provides patient assessment, counseling, and discharge planning.
• Acts as liaison between patient/family/hospital and community resources.
• Works as part of an interdisciplinary team with physicians, nurses, and case management.
• Utilizes psychosocial interventions and social psychotherapy.
• Occasionally plans educational offerings and in-services as requested.

Education

Bachelor’s Degree in Social Work

Master of Social Work preferred
