Job summary

The Meadows of Central Massachusetts, a Vibra Healthcare facility, is seeking a Part Time Social Worker to join our team. You will provide patient assessment, counseling, and discharge planning within an inter-disciplinary team to support patients and families.

Minimum qualifications include a Bachelor’s Degree in Social Work and an active license in Massachusetts; a Master of Social Work is preferred. Prior healthcare experience is valued, along with strong organizational and computer skills.

Qualifications

• Bachelor’s Degree in Social Work is required.
• Current, valid social work license in the state of employment is required.
• Master of Social Work preferred.
• Two years of healthcare social work experience preferred.
• Strong organizational and analytical skills; professional image.
• Working knowledge of computer and software applications used in job functions.

Responsibilities

• Provide patient assessment, counseling, and discharge planning.
• Act as liaison between the patient/family/hospital and community resources.
• Work as part of an inter-disciplinary team with physicians, nurses, and case management.
• Apply psychosocial interventions and social psychotherapy.
• Occasionally plan educational offerings and provide in-services as requested.
