Job summary

PeaceHealth in Oregon is seeking a Clinical Social Worker for a part-time, 0.80 FTE, day position. The role coordinates patient care through diagnosis, clinical assessment and treatment planning within a multidisciplinary team.

Responsibilities include intake and assessment, discharge planning, case management, supervision of students, and ensuring privacy laws compliance. Licensure as LCSW/CSWA/LMSW is required, with ongoing supervision and quality improvement involvement.

Qualifications

• Master's in Social Work from an accredited program.
• Licensed/credentialed social worker required (OR).
• Experience with hospital/clinical settings preferred.

Responsibilities

• Provide intake, ongoing assessment and treatment planning with the care team.
• Manage multiple complex cases and coordinate discharge plans.
• Deliver brief therapy and participate in team staffings with physicians.
• Ensure privacy/compliance; interface with legal entities as needed.
• Support quality improvement through monitoring of services.
• Supervise social work students and CSWA licensure supervision.
• Diagnose/address mental health and chemical dependency needs; perform risk assessments.
• Screen for care management per high-risk criteria.
• Develop and monitor comprehensive discharge plans with the team.
