Job summary

Impact Life is seeking a credentialed Intensive Outpatient Counselor to join our Social Prescribing team in Newark, Delaware. This part-time role provides evidence-based individual, group, and family therapy within the Intensive Outpatient Program.

Responsibilities include assessments, treatment planning, counseling, care coordination, and compliant clinical documentation. Schedule is up to 18 hours weekly, typically Monday, Tuesday, and Thursday 8:00 AM–1:00 PM, with some flexibility for

Qualifications

• Bachelor's degree in Social Work, Counseling, Psychology or related behavioral health field required.
• Active Delaware CADC credential required.
• Knowledge of behavioral health treatment and evidence-based practices.
• Strong clinical assessment, communication, documentation and interpersonal skills.
• Ability to work independently while collaborating on an interdisciplinary team.

Responsibilities

• Provide individual, group, and family counseling within the IOP.
• Facilitate evidence-based therapeutic groups for substance use and co-occurring disorders.
• Conduct biopsychosocial assessments and ongoing clinical assessments.
• Develop individualized treatment and recovery plans based on client needs.
• Monitor progress and adjust treatment plans as clinically appropriate.
• Coordinate care with case managers, medical providers, and community partners.
• Maintain accurate clinical documentation in the EHR system.
• Participate in multidisciplinary team meetings and quality improvement activities.
• Discharge planning and referrals to continuing care services.
