Akron Children's Hospital is seeking a Clinical Pharmacist I to join the Inpatient Pharmacy team in Akron, OH. The role provides centralized services and supports antimicrobial stewardship, anticoagulation monitoring, medication safety, and pharmacotherapy optimization.

Responsibilities include dispensing, formulary management, collaboration with clinicians, patient education, and EMR documentation. Part-time, 16 hours/week, with shift rotations and onsite work in Akron.

Formación

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college
• Current license by the Ohio State Board of Pharmacy
• Valid State driver's license and ability to obtain and maintain insurability
• Completion of PGY1 residency or equivalent years of experience preferred
• Pediatrics experience preferred

Responsabilidades

• Review, verify, and dispense medications in accordance with hospital policies, state regulations, and clinical best practices.
• Promote cost-effective medication use through formulary management, stewardship initiatives, and optimization of medication utilization.
• Provide pharmacy services in centralized and decentralized areas as assigned.
• Collaborate with physicians, nurses, and other healthcare professionals to optimize medication therapy outcomes.
• Provide drug information and pharmacotherapy recommendations to healthcare staff and patients.
• Document clinical interventions and patient care activities in the EMR and pharmacy systems.
• Participate in medication safety, quality improvement, and regulatory compliance initiatives.
• Precept pharmacy students, residents, or new staff as assigned.
• Monitor medication therapy for appropriateness, dosing, duration, results, and opportunities for clinical intervention.
• Other duties as required.
