Job summary

Forward Family Care, LLC is seeking an in-home youth care worker to support young people, including those with special needs, in home and community settings around Jefferson County, WI. This role emphasizes individualized care plans, behavioral health support, and fostering growth.

You will coordinate with families and professionals, collect progress data, and participate in ongoing training. Part-time, in-person position with flexible afternoon and weekend hours to meet client needs.

Qualifications

• Experience or education in special education, social work, or related fields is preferred.
• Knowledge of behavioral health practices for youth with diverse needs.
• Strong communication skills, both verbal and written.
• Familiarity with data collection methods to track progress.
• Experience in home community care settings is a plus.
• Ability to work collaboratively and independently.

Responsibilities
