Job summary

Forward Family Care, LLC seeks an in-home youth care worker to support young individuals, including those with special needs, in Jefferson County, Wisconsin. The role offers a flexible workload and the chance to work with multiple clients, both at home and in the community.

Responsibilities include implementing care plans, promoting social and behavioral development, and coordinating with families and professionals. A high school diploma and driver’s license are required.

Qualifications

• Experience in caring for youth or children, especially with diverse needs.
• Strong communication skills, both written and verbal.
• Knowledge of behavioral health approaches for youth development.
• Experience using data collection to track progress and outcomes.
• Ability to collaborate with families and professionals in a team.
• Capability to work independently when needed.

Responsibilities

• Provide direct support to youth in home and community settings.
• Implement individualized care plans focusing on behavioral health and social skills.
• Collect data to monitor progress and adjust care strategies.
• Facilitate communication among youth, families, and care team.
• Support youth with autism using specialized engagement techniques.
• Collaborate with social workers and educators to build supportive networks.
• Participate in ongoing training to improve caregiving and education skills.
