Forward Family Care, LLC is seeking an in-home youth care worker to support children and adolescents with diverse needs in Dane County, WI. The role involves delivering direct care at home and in community settings, following individualized care plans and promoting social skills and personal growth.

• Experience or education in special education, social work, or related fields is preferred.
• Knowledge of behavioral health practices and techniques for working with youth with diverse needs.
• Strong communication skills, both verbal and written, with the ability to connect with youth effectively.
• Familiarity with data collection methods for tracking progress in behavioral interventions.
• Experience in home community care settings is a plus.
• Ability to work collaboratively within a team environment while also being able to work independently when necessary.
• A passion for helping young individuals thrive and succeed in their personal development.
