Job summary

Qualifications

• Experience or education in special education, social work, or related fields preferred.
• Knowledge of behavioral health practices for youth.
• Strong communication skills, both verbal and written.
• Familiarity with data collection methods to track progress.
• Experience in home/community care settings is a plus.
• Ability to work collaboratively and independently as needed.

Responsibilities

• Provide direct support to youth in home and community settings.
• Implement individualized care plans focused on developmental goals.
• Engage in data collection to monitor progress and adjust care strategies.
• Facilitate communication between youth, families, and professionals.
