Job summary

Signify Health, part of CVS Health, seeks a Part Time Physician to perform In-Home Health Evaluations, engaging patients at their homes and bridging gaps in care.

You’ll connect with health plan members, review medications and medical history, conduct a physical evaluation, and offer diagnostic services as needed. Travel locally and within licensed states is required; virtual visits may be conducted.

Qualifications

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses preferred.
• Part-time physician role with in-home evaluation responsibilities.

Responsibilities

• Provide in-home health evaluations for members.
• Travel locally to patient homes and to licensed states as needed.
• Perform med history review and physical evaluations, with optional virtual visits.
• Utilize iPad and clinical workflow to document evaluations and ensure quality care.
