Job summary

Signify Health, part of CVS Health, is seeking a Part-Time Physician to deliver in-home health evaluations, visiting patients at their residences and helping bridge gaps in care.

The role involves 29.5 hours per week, travel within licensed states, and use of an iPad for a streamlined clinical workflow. Candidates must hold an active medical license and be able to work in home settings.

Qualifications

• MD or DO credential required.
• Active medical license in Illinois.
• Multi-state licenses preferred.

Responsibilities

• Perform in-home health evaluations with patients in their own homes.
• Engage patients to bridge gaps in care and connect to appropriate services.
• Use iPad-based clinical workflow to document findings.
• Travel locally and within licensed states as required.
• Conduct virtual visits when needed.
• Provide Diagnostic & Preventive Services per plan.
