Job summary

Signify Health, part of CVS Health, is seeking a part-time clinician (NP or PA) to provide in-home health evaluations. The role involves visiting members at home, performing physicals and history reviews, and coordinating care.

You will use an iPad-based workflow and may conduct virtual visits as needed. Qualified candidates hold an active, unrestricted NP or PA license, board certification, and multi-state licenses are preferred.

• Active, unrestricted license(s) in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Conduct In-Home Health Evaluations for health plan members.
• Build trusted relationships and bridge gaps in care.
• Use iPad and clinical workflow to perform evaluations.
• Travel locally to visit members in their homes as needed.
• Perform virtual visits when required.
• Provide Diagnostic & Preventive Services as indicated by protocols.
