Responsibilities

• Conduct in-home health evaluations for plan members.
• Travel locally and within licensed states; conduct virtual visits as needed.
• Provide Signify Health services including Diagnostic & Preventive Services (e.g., spirometry).
• Collaborate with clinicians to ensure quality and safety during visits.
