Job summary

Union Gospel Mission of Tarrant County in Fort Worth, TX seeks a part-time Case Manager to oversee daily program planning and resident services. You will support residents in developing life skills, emotional healing, and spiritual growth, helping them transition to independent living within a Christ-centered setting.

The role requires five years of clinical Social Work/Case Management, a Bachelor in Social Work or Ministry, and LMSW/LBSW licensure preferred.

Qualifications

• Five (5) years of clinical Social Work/Case Management experience preferred.
• Bachelor in Social Work or Ministry.
• Texas LMSW preferred; LBSW allowed.

Responsibilities

• Screen and perform psychosocial assessment for incoming residents.
• Complete new resident intake process, including intake assessment.
• Develop Care Plans and Individualized Goals with residents; monitor and document progress.
• Provide residents with assistance regarding program progress, attendance, and issues.
• Lead educational courses, developmental groups, and resident meetings.
• Coordinate referrals with outside agencies and resources.
• Maintain documentation for all program services provided.
