Union Gospel Mission of Tarrant County in Fort Worth, TX seeks a Case Manager to oversee daily program planning and implementation for a caseload of Residents, helping them transition to independent living by supporting skill development, emotional healing, and spiritual needs.

Responsibilities include psychosocial assessments, intake, care plans, documenting progress, leading classes, connecting residents to community resources, and coordinating discharge plans with peers.

Qualifications

• Five years of clinical Social Work/Case Management experience.
• Experience serving unsheltered communities, veterans, and families.
• Bachelor's in Social Work or Ministry (direct experience case managing homeless persons can be considered in lieu of education).
• Texas LMSW preferred; Minimum LBSW.
• Proficient in MS Word, Excel and Outlook; ability to work occasional flexible hours.

Responsabilités

• Screen and perform psychosocial assessment for incoming residents.
• Complete new resident intake process, including intake assessment.
• Develop Care Plans and Individualized Goals with residents; monitor and document goal and care achievements.
• Provide residents with assistance and support regarding program progress, class attendance, and behavioral issues.
• Lead educational courses, developmental groups, and resident community meetings.
• Connect residents to referral network/linkages with outside agencies and resources.
• Maintain documentation for all phases and services provided.
• Interact with residents on life skills, daily living activities, laundry, room cleanliness, hygiene, etc.
• Develop, review and update a Discharge Plan and Exit strategy with residents.
• Participate in cross functional support and collaboration for residents and peers.
• Other duties as assigned.
